UK military instructor filmed own death on GoPro when parachute failed during 12,000-foot training jump

A UK military instructor unwittingly filmed her own death when her parachute failed to open during a 12,000-foot training jump.

Veteran skydiver Sergeant Rachel Fisk, 32, was snapped in her final jump as she used a GoPro on her helmet to film a student and instructor making a tandem jump after leaping from a Cessna over Oxfordshire.

After the pic was taken, Fisk’s own chute failed to operate — leaving her plunging at least 5,000 feet to her death, hitting speeds of up to 120 mph, according to the Daily Mail.

The Royal Air Force (RAF) initially assumed its instructor had landed safely, until a taxi passenger called emergency services to report “seeing someone falling from the sky,” the paper said.

Now, a Defence Safety Authority Service report has revealed that officials only discovered what went wrong in the 2021 tragedy when they reviewed Fisk’s GoPro footage.

It showed her looking up to see her chute entangled — with the last frame taken just 0.034 seconds before impact, the report said.

“It was a freak accident, an absolute tragedy,” a senior RAF source told the outlet.

“I hate to think what Rachel’s last thoughts were.”

Investigators believe the parachute failed because of air turbulence produced by her body position during free fall, with the wings on her non-military-issue jumpsuit possibly also getting in the way, the Mail said.

At the time of her death, Fisk’s family said that “she lived her life with joy and thoughtfulness for others and loved the career she had chosen.

“Rachel will be missed by so many.”
